Il personale di supporto di NHibernate mi ha appena indirizzato nella giusta direzione per risolvere il problema.

Il problema era che IIS non disponeva delle autorizzazioni per accedere alla cartella windows \ temp.

sono andato alle IIS AppPool> settigns avanzate> Identità

E cambiato in LocalSystem (dato che sono l'admin il mio PC). Questo lo ha risolto per me. Spero che questo ti aiuti.
